Welcome to the Command Center for Saint Luke’s Hospital In Your Home! This room is where health care providers remotely assist patients by answering calls, providing virtual check-ups, monitoring vitals, overseeing medications, and more.

Hospital In Your Home is an innovative, patient-centered program that brings hospital-level care to patients in the comfort of their own homes through in-person and virtual care. Patients are given a technology kit that includes a tablet and phone, which they can use 24/7 to communicate with their care team. In its first two years, the program has cared for nearly 1,300 patients.
